The Family's Perspective

When Mrs C was discharged from The Royal Marsden Hospital following extensive surgery, her family needed reassurance that she would be safe and supported at home. The surgery had resulted in the formation of a stoma, and Mrs C faced a challenging recovery — adjusting to life with a new stoma, managing post-operative wounds, and coping with significantly reduced mobility due to severe pain. Her sons, who live nearby, contacted HP Homecare to arrange 24-hour nursing care. With discharge expected within days, the family needed a nurse who could provide stoma support, personal care, mobility assistance, and wound management — and who could start at short notice.

Coordinating a Smooth Discharge — Michele's Role

Michele, our Registered Nurse Manager, received the referral and visited The Royal Marsden to meet Mrs C whilst she was still an inpatient. She liaised closely with the hospital staff, took instructions from the clinical team, and worked alongside them to ensure a smooth, well-co-ordinated discharge. This in-person handover meant that when Mrs C returned home, our team had a clear understanding of her care plan, her wounds, her stoma requirements, and the support she would need. The hospital team were impressed by the level of co-ordination, and the family felt confident that everything was in place.

Care at Home — Nokkie's Role

Torie described Nokkie as “gentle, kind and very experienced” — here's how that showed in Mrs C's care.

Our nurse, Nokkie, assumed responsibility for Mrs C's care upon discharge. She conducted the initial assessment in Mrs C's home, ensuring she was supported and not left alone during the early stages of her recovery. Mrs C's care plan included the administration of post-operative prophylactic anticoagulation, stoma care and appliance management, wound care for several sites including a skin graft, and psychological support to assist her emotional wellbeing during this challenging time.

Nokkie co-ordinated multidisciplinary input, including district nurses, stoma care nurses, and an occupational therapist — relieving Mrs C and her family of the logistical and practical concerns associated with managing complex care needs whilst recuperating. A strong bond developed between Mrs C and Nokkie, who agreed to work continuously for several weeks to provide consistency and continuity.
